FUNCTIONAL CAPACITY EVALUATIONS

A Return to Work program (RTW) is developed by working collaboratively with the client, employer, case manager and health care providers.  This service can include the following:

· On-Site Work Evaluation and Education

· Recommendations for Work Site Modifications and Adaptations

· Meetings with Parties Active on File

· Regular Meetings with Client and Employer

· Structured Return to Work Plan

 

COST: $90.00/hour

RETURN TO WORK PROGRAMS

Case Management Services can be used to assist in the management of a file or for specific task assignment on a file.  This service can include the following:

· File Review

· Gathering of Medical Documentation

· Meetings with Client and Healthcare Providers Active on File

· Development and Implementation of a Rehabilitation Plan

· Arranging Assessments and Rehabilitation Services

· Return to Work Plan

 

COST: $90.00/hour

CASE MANAGEMENT

Independent Medical Evaluations can be arranged with appropriate specialists such as:

· Orthopedic Surgeons

· Physiatrists

· Anesthesiologists

ERGONOMIC ASSESSMENTS

A Job Site Analysis is an objective evaluation of the demands of a particular job.  This evaluation includes information on the following:

· Mobility/Body Position Requirements

· Strength Requirements

· Environment/Ergonomic Risk Factors

· Sensory/Cognitive Demands

· Alternate/Modified Work Availability

An Ergonomic Assessment is an evaluation of an individual’s workstation, work methods, and work environment to determine if there is a good fit between the work and the worker.  This assessment includes the following:

· Assessment of Posture and Body Mechanics

· Assessment of Workstation and Work Tool Design

· Assessment of Work Environment

· Education

· Recommendations to Increase Worker Comfort, Increase Productivity, and Decrease Risk of Injuries

A Transferable Skills Analysis (TSA) is an evaluation of the skills that transfer from an individual’s work history for the purpose of determining alternate employment options considering the individual’s physical abilities.  A Transferable Skills Analysis includes the following:

· Transferable Skills Analysis and Job Search

· Labour Market Information Search

· Report

A Vocational Evaluation is a formal evaluation of an individual’s skills, aptitudes, academic achievement levels, and interests for the purpose of determining alternate employment options and/or retraining potential.  A Vocational Evaluation includes the following:

· File Review

· Client Interview

· Formal Evaluation

· Analysis of Data Obtained and Job Search

· Labour Market Information Search

· Report

 

 

COST: $2495.00

Additional charges apply if travel outside of Moncton is required.  Also, for individuals that require the evaluation to be spread out over more than the typical 3 day period, the additional fee is $250/day.  Please call for quote.

TRANSFERABLE SKILLS ANALYSIS

VOCATIONAL EVALUATIONS

Cognitive Assessments are used to provide baseline measures, to assist in defining areas of focus for cognitive rehabilitation, and to evaluate treatment effectiveness.  Cognitive Assessments can provide measures for the following:

· Orientation

· Attention

· Memory

· Executive Functions

· Perception

 

A Cognitive Demands Analysis of an individual’s job can also be performed.  This is a very helpful tool when determining the return to work potential for an individual with cognitive impairment(s).

Cognitive Rehabilitation is a form of rehabilitation used with individuals who have cognitive impairments for various reasons, such as traumatic brain injuries, brain tumors or strokes. Various techniques are used in the rehabilitation process, including the following:

· Cognitive Training Exercises to Regain Lost Function

· Training in the Use of Assistive Devices and Cognitive Strategies to Substitute for Lost Function

· Changing Environmental and Task Demands to Optimize Functional Performance

The Occupational Therapist can also develop return to work plans and strategies considering an individual’s cognitive abilities and the cognitive demands of their job.

COGNITIVE AND PERCEPTUAL ASSESSMENTS

COGNITIVE REHABILITATION

A Home Assessment is completed to identify any safety concerns and/or barriers to optimal functioning in an individual’s home environment.  This assessment may include the following:

· Evaluation of Home Environment

· Evaluation of the Methods and Tools Used by an Individual in Their Home Environment

· Education on Topics Such as Posture, Body Mechanics, and Energy Conservation

· Recommendations for Environmental Modifications, Equipment, Tools, and/or Assistive Devices

Comprehensive Activity of Daily Living Assessments are also available.  These assessments identify pre verses post accident functioning and provide recommendations for achieving optimal functioning and compensating for lost function.

The Progressive Goal Attainment Program (PGAP) is a 10 week standardized and cost effective treatment program aimed at reducing pain related disability.  The main components of the program are as follows:

· Activity Log

· Activity Scheduling

· Walking Program

· Increasing Activity Involvement

· Overcoming Psychosocial Obstacles to Activity Involvement

· Evaluating the Benefits of PGAP

 

An individual being considered for this program will be evaluated prior to commencing the program to ensure they are a good candidate for PGAP.  Further information on the PGAP program can be found at http://www.pdp-pgap.com

 HOME ASSESSMENTS

Health and Wellness Education provides information to employees on subjects that are important for a worker’s health and productivity.  The goal is injury prevention, health promotion, and productivity.  Topics include:

 

· Stress Management

 

· Ergonomics

 

· Safe Work Techniques

 

· Chronic Pain Management

 

 

COST:  $225 for 1 hour session plus $2/  

              person for handouts

             $450 for 3 hour session plus $2/
